Each Coach or Team Manager MUST Present (At the First League Game)

  • A "New Individuals" form for each person not yet registered at Sports Underdome.

    • Headshot Photos will be taken during the first two weeks of competition.

  • One "Teams with Existing Members" form listing the names and ID#s of previously registered players. 

  • A $10 cash or check payment (payable to Gemma-Tech) for each

    • Not-Yet-Registered individual

    • Registered individuals whose ID is lost

    • Registered individuals whose registrations are not valid for the entirety of the league.
 
Notes about Sports Underdome Registration and ID Cards

  • All players and coaching staff must present a valid Sports Underdome ID Card at every league game.

  • All individuals that have participated in a Sports Underdome league since the Winter '06 session were issued an ID card and member ID#. 

    • These individuals should NOT complete a "New Individuals"

    • Rather, they should list their information on the team's, one "Teams with Existing Members" form.

  • Any individual that has not participated in a Sports Underdome league since the Winter '06 session must complete the "New Individuals" form below and attach $10 cash or check payable to Gemma-Tech.

    • Headshot Photos will be taken during the first two weeks of competition.

  • If you have a Sports Underdome ID Card that is not valid for the entirety of the league in which you will be participating, you must renew your membership. 

    • Please DO NOT complete a "New Individuals" form

    • Rather, fill out the appropriate information on the "Teams with Existing Members" form and attach $10 cash or check payable to Gemma-Tech for each member.
